*Qualifications:*
* Must have an active state license in Physical Therapy and meet the educational requirements for that license type
* 2 plus years of experience in Skilled Nursing Facility Rehabilitation or Outpatient Rehabilitation is preferred
*Responsibilities:*
* Manage all activities of the Rehabilitation Department, including clinical assessments, treatments, documentation, compliance, and billing
* Provide high-quality Rehabilitation Services in the licensed field (Physical Therapy)
* Coordinate admissions of residents requiring skilled rehab services
* Create Outpatient services and work together with the Director of Outreach in the Marketing Department, overseeing Skilled Nursing, Independent Living, and Outpatient Rehabilitation.
* Evaluate, develop, organize, and implement resident care programs utilizing selected treatment procedures in individual and group sessions.
* Determine and schedule staff daily based on patient needs to meet regulatory requirements and efficiency standards.
* Coordinate rehabilitation requirements and the plan of care for each resident.
* Conduct patient referral marketing in collaboration with admissions programs.
* Ensure completion of required quality assurance and program evaluation activities.
* Manage HR functions, including recruitment, performance reviews, and disciplinary processes.
* Oversee own caseload, identifying residents through screening processes and scheduling treatment while maintaining efficiency expectations.
* Provide direction to rehabilitation personnel and assist in the supervision.
* Provide orientation, mandatory in-services, continuing education, and other training to meet department and regulatory requirements.
* Adhere to ethical, regulatory, procedural, and professional standards and practice guidelines
