*SUMMARY:*
To assist the Program Director of Adult Family Care and Shared Living in overseeing the residential services provided in the Shared Living program.
Ensuring the effective delivery of services specified in each individual’s support plan.
Protects and enhances rights of individuals and promotes individuals as capable, valued, and accepted members of the community.
Additionally, supports the Director with Case Management and support of AFC (Adult Family Care) members.
Ensures monitoring and updating of Icentrix files, as well as maintaining a tracking system for all MassHealth paperwork.
Complies with all principals of Human Rights, DPPC, Department of Developmental Services, the division of medical assistance, MassHealth, and all other applicable funding regulations as adhered to by Lifeworks.
*ESSENTIAL DUTIES AND RESPONSIBILITIES:* (include the following.

Provide Case Management to AFC and Shared Living families, Providers, and Individuals served.
Responsible for managing case files and attending ISPs.
Scheduling monthly visits for AFC and Shared Living
New provider screening and placement of new referrals for shared Living.
Create and update a database of Shared Living providers.
Support AFC to transition to Icentrix.
Create a tracking system for MassHealth requirements.
Create a relief pool/respite for Shared Living.
Attending bi-weekly staff meeting with AFC and Shared Living.
To provide support services to families in a way that recognizes and respects their ethnic and cultural diversity.
Strong written and verbal communication skills including excellent interpersonal skills.
Other duties and/or assignments to work/program locations based on the needs of the company.
Regular scheduled M – F workweek and evenings and weekends to be available when needed.
Assist Community Living Director in the planning, development, coordination, and management of the AFC and Shared Living programs.
In the absence of the Director, assume the Director’s duties.
On call duties for Shared Living – after hours and weekends to receive updates and support to individuals and provider.
Assists in provision of emergency support as needed.
May be asked to work evenings, weekends.
*SUPERVISORY RESPONSIBILITIES:*
Supports the oversight and operation of the AFC and SL programs in the absence of the Program Director.
*Education and Experience: *
* Bachelor's degree from four-year college or university and minimum of three years related experience, with supervisory experience preferred.
